We would like to thank you for scheduling an appointment.  We look forward to providing you with the best dental treatment available.

Our normal working hours are Monday-Thursday, 8 a.m. - 1 p.m. and 2 p.m. - 5 p.m., and typically every other Friday 8 a.m. - 2 p.m.  In the event you experience a dental emergency outside of our business hours, our answering machine will inform you of how to reach Dr. Schuster.

Below you will find links to our new patient forms which needs to be filled out.  The first is a patient information form which needs to be completed and signed on the front.  The second form is a medical history.  Please complete the information and sign at the bottom.  On the back of this form you will need to complete questions about your dental health and sign at the bottom.
